North Wales Memory Support Pathway – Alzheimer’s Society Involvement awards 2024

Alzheimer’s Society strive to ensure people living with dementia are at the heart of everything they do. Understanding that when individuals with lived experience are consulted, involved, and co-produce services, products, or activities, it yields the greatest impact. Annually, Alzheimer’s Society recognise individuals and teams who have gone above and beyond to involve and include those with lived experience. At the 2024 Alzheimer’s Society Involvement awards 2024, the North Wales dementia support team achieved a “Highly Commended” award.

Commissioned by North Wales Regional Partnership Board, Alzheimer’s Society partner with NEWCIS, Carers Trust and Carers Outreach to co-produce and deliver the “North Wales Memory Support Pathway”. The team was recognised for being determined to make a difference every day and actively seeking ways to help and support people living with dementia and their unpaid carers.

The Memory Support Pathway work encouraged third sector organisations to work together, contributing what they do best rather than competing against each other. Click here for more information about the Memory Support Pathway.
